The attack claimed the lives of Bebe King, Alice da Silva Aguiar, and Elsie Dot Stancombe, and left ten others injured. The inquiry revealed that Rudakubana had been referred to the counter-terror agency Prevent three times but was dismissed each time, indicating a troubling pattern of agencies passing responsibility for cases like his. “If the full extent of [Rudakubana’s] family’s concerns had been shared with authorities… it is almost certain this tragedy would have been prevented,” Fulford emphasized.
Rudakubana’s violent behavior was not a new development; he had previously taken a knife to school and attacked another pupil. His parents, however, failed to report their concerns about their son’s actions to the authorities, which may have contributed to the lack of intervention. The inquiry’s findings suggest that a more proactive approach could have mitigated the risks associated with Rudakubana.
In the aftermath of the attack, the inquiry’s report, spanning 700 pages, highlighted a culture of negligence among the agencies involved. Fulford criticized the lack of accountability, stating, “The frankly depressing – and therefore urgent – matter requiring government attention is this failure… to stand up and accept responsibility for managing the risk that [Rudakubana] posed.” He further asserted, “Rigorously putting out of mind the so-called ‘benefits of hindsight’, I have no doubt that if appropriate procedures had been in place… this dreadful event would not have happened.”
